Liability Waiver: I understand and acknowledge that there is a risk of injury inherent in dance activities and that personal injury or damage to property may result during participation in dance classes, camps, and related activities. I represent that my child is physically able to safely participate in camps, dance, and related activities. I agree to assume all risks associated with my child's participation and I release and hold harmless CC & Co. Dance Complex, its owners, administrators, and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained while in or upon the premises or any premises under the control and supervision of CC & Co. Dance Complex. In consideration, of attending and participating in all the activities included in enrollment at CC & Co. Dance Complex, I hereby waive, release, and discharge all present and future claims and liabilities of any kind, whether for bodily injury, death, property damage, or other loss, arising out of my child's participation in any and all related activities. This also includes but not limited to camp activities, dance instruction, rehearsals, and performances, and events whether conducted on or off CC & Co. Dance Complex premises or en route to or from said premises. I also acknowledge that I am responsible for delivering my child to the studio and picking her/him up and that the studio is not responsible for a child that leaves the premises *
